The opportunity:

Responsible for the interdisciplinary direct and indirect patient care across the care continuum, with an emphasis on outpatient care, for a designated population (population health). This position requires expertise in a systems approach to problem solving, using critical thinking skills and knowledge of community resources to plan and coordinate care.

What you’ll do:

Operational Excellence
  • Develop, implement, and evaluate a Care Management program resulting in an exceptional patient experience, improved health of our member populations, and reduced annual medical spend.
  • Responsible for strategy and service design to ensure coordination of care across the continuum and population health management, for low, medium, and high risk patients.
  • Plan, select and devise care coordination operations, work methods, procedures, workflows and standards for quality and quantity of work, including staffing standards.
  • Recognize and respond to patient care issues providing direction, feedback, and follow-up to staff or other healthcare team members.
  • Monitor and evaluate operations for compliance with licensure, accreditation and regulatory standards.
  • Provides management, coordination and oversight for implementation of new B2B partners within Care Management, including the set-up, training, launch, and post-launch support.
  • Establish partnership with project management during weekly meetings, new project kickoffs, implementation, post implementation management
Products/Services
  • Oversees and coordinates deployment of products necessary for critical expansion of services, including referral management secondary care options
  • Coordinates with marketing team to develop marketing material according to established product parameters
Leadership/Strategy
  • Leads development and implementation of a strategic plan that supports Care Management vision and goals; translates the strategic plan into Care Management to staff to ensure support<span data-offset-key="7108f-5-0">; modifies or adjusts the plan in response to changing internal and external factors
  • As part of the Clinical Operations leadership team, develops and deploys policy and strategy to meet or exceed revenue, profit and cash flow commitments
  • Facilitates the application of the tools, processes, etc to enable design and deployment of consistent organizational processes and systems to enable cross-functional strategy deployment within Clinical Operations
  • Participates with the Operations leadership team in setting policies and forming strategies to meet or exceed revenue, profit and cash flow commitments to the company, specifically incorporate / optimize the financial performance of single unit levels (reducing fixed costs, managing variable labor costs, projecting expenses, etc)
  • Analyzes Care Management current and future trends and needs and accurately assesses functional responsibilities, competitive strengths and vulnerabilities
  • Upholds, safeguards and promotes the organization’s values and philosophy relating to ethics, integrity, and corporate responsibility as referenced in organizational values and philosophy standards
  • Designs and oversees deployment of Care Management departmental structures, operations systems and practices and are responsive and adaptable to evolving business needs.
  • Develops and implements a Care Management talent plan to ensure that the right people are in the right lace at the right time to meet the strategic needs of the organization
  • Manages Care Management employee performance by establishing clear goals and expectations, tracking progress against goals, ensuring timely feedback and addressing performance problems and issues promptly

What you’ll need:

  • Registered Nursing license or Licensed Clinical Social Worker
  • Three (3) to five (5) years clinical experience required
  • Three (3) years management experience preferred
  • Masters degree in Nursing, Social Work or other healthcare related field, preferred
  • Experience managing care management or case management within a telehealth or digital health organization.
  • Proven leadership experience that includes: team building, motivating staff with enthusiasm and compassion, communicating skills across disciplines.
  • Ability to lead multi-disciplinary teams while implementing and expanding a care management program.
  • Strong communication, customer service and decision –making skills; knowledge of budgetary processes and financial principles<span data-offset-key="cjed2-4-0">; knowledge of labor laws, standards of care, and regulatory standards required.
  • Willingness to perform Care Manager duties as needed.
